Where deemed medically appropriate, your clinician can review your ongoing medications and issue repeat prescriptions. Options are discussed openly before any prescription is issued.

With your consent, your clinician can share a summary with your regular GP and arrange referrals to specialists or allied health, so everyone involved in your care stays informed.

What to expect

What to expect from your consultation.

  1. Before Your Consult

    Complete the intake form with your medical history, current medications, recent results and any concerns. This allows your clinician to prepare for your review.

  2. During Your Consult

    A video consultation where your clinician reviews how you are tracking, discusses any changes and works with you on a care plan suited to your circumstances.

  3. After Your Consult

    Your care plan, prescriptions and referrals are delivered electronically. With your consent, a summary can be sent to your regular GP so your records stay up to date.

  4. Ongoing Support

    Living with a long-term condition is an ongoing journey. Regular follow-up consults allow your clinician to review your progress, adjust your plan and provide continued support.

Living with a long-term condition is easier when your care is regular, organised and coordinated. Our chronic disease management consult pairs you with an AHPRA-registered clinician who takes the time to understand your health history, current medications and how you are tracking, then works with you on a plan that suits your circumstances.

Your clinician conducts a clinical review and develops a care plan with you. Where deemed medically appropriate, your medications can be reviewed and repeat prescriptions issued, with the options discussed openly. With your consent, referrals to specialists or allied health can be arranged, and a summary shared with your regular GP, so everyone involved in your care stays informed. Regular follow-up consults allow your clinician to review your progress and adjust your plan over time.

Telehealth is not suitable for all situations. Your clinician may recommend in-person assessment, GP review, specialist referral, further investigation or no treatment depending on your circumstances. Individual results vary. All clinical decisions are made by your treating clinician based on your personal health profile.

Chronic Disease Management: Common questions

No. Our service is designed to complement your existing care, not replace it. With your consent, we can send a summary to your regular GP after each consultation so your full health picture stays coordinated.
Where deemed medically appropriate after a clinical review, your clinician can issue repeat prescriptions for your ongoing medications. This is determined on a case-by-case basis.
This depends on your circumstances and the plan you develop with your clinician. Some people benefit from regular check-ins, while others need them less often. Your clinician will recommend a follow-up schedule based on your needs.
Our clinicians support adults living with a range of ongoing conditions, including type 2 diabetes, high blood pressure, asthma, COPD and chronic kidney disease. If telehealth is not suitable for your situation, your clinician will let you know and recommend the right next step.
Where clinically appropriate, your clinician can arrange pathology or imaging referrals to complete at an accredited facility near you. If an in-person review is needed, they will guide you on the right next step.
